Unusual Case of B-Chronic Lymphocytic Leukemia with Coexistence of Precursor T-Acute Lymphoblastic Leukemia Status Post Transplant

چکیده
Background: Coexistence of Chronic lymphocytic lymphoma (CLL) to Acute lymphoblastic leukemia is extremely rare with only a few reported cases [1-5]. Chronic lymphocytic leukemia is a neoplasm of small monomorphic round to irregularly shaped lymphocytes of B-cell lineage. Adverse prognostic factors in CLL include flow cytometric expression of ZAP-70 and CD38 which are surrogate markers for the mutational status of IVIg. T-cell acute lymphoblastic leukemia (T-ALL) is a neoplasm of lymphoblasts that are of T-cell lineage and are small to medium-sized cells. Case Report: We present a case of a 62-year-old man, diagnosed with B-Chronic Lymphocytic Leukemia who was treated successfully twice. A year following an allogeneic stem cell transplantation (SCT), the CLL returned for a third time along with a secondary diagnosis of T-cell acute lymphoblastic leukemia. Our review of literature explores the potential pathophysiologic mechanisms of the coexistence of these two diagnoses. Conclusion: Chronic lymphocytic leukemia has been known to evolve into various lymphoid transformations, including diffuse large b-cell lymphoma, and prolymphocytic leukemia, however the coexistence and transformation of CLL to T-cell acute lymphoblastic lymphoma is extremely rare. Potential mechanisms of this phenomemon may include a common genetic predisposition, a malignant transformation of a common progenitor cell, immune deregulation, chronic stimulation of T-cells by the B-CLL cells, adverse effects of chemotherapy exposure, increased T-cell resistance to apoptosis, or to the slight occurrence of pure chance. Numerous hypotheses exist, however the precise relation of the two entities coexisting is unknown due to limited research and only a few known cases. Open Journal of Hematology

Face Bones Involvement and Relapse in a Case of Childhood Acute Leukemia
Midface bones are an unusual site for primary presentation and relapse in acute lymphoblastic leukemia. Herein, we describe a case of acute pre B cell lymphoblastic leukemia with leukemic infiltration of maxilla and bone marrow involvement. At the time of relapse, the patient presented again with maxilla involvement and the phenotype changed to biphenotypic lymphoblastic leukemia. Our case sug...
متن کاملPediatric B Cell Acute Lymphoblastic Leukemia presenting with Paraneoplastic Acute Disseminated Encephalomyelitis
Acute Disseminated Encephalomyelitis (ADEM) is a monophasic demyelinating disease most often triggered by infection or immunization, though associations with malignancy and stem cell transplant have been described. We described the case of a four-year-old boy with new-onset neurological symptoms associated with ADEM, acute leukemia, and equivocal evidence of Mycoplasma pneumoniae infection. He ...
متن کاملAcute Lymphoblastic Leukemia Following Nasopharyngeal Carcinoma: Report of an Unusual Case
Introduction: Nasopharyngeal carcinoma (NPC) is a rare malignancy in children. Nasal obstruction, otitis media with effusion, pain in the ear, hearing problems, and unusual neck mass are among the signs and symptoms of this malignancy. Case Report: We report the case of a 13-year-old girl with NPC who later developed acute lymphoblastic leukemia (ALL) through the course of her disease. To our...

Background: Precursor B-Acute Lymphoblastic Leukemia (precursor B-ALL) oc-curs due to the uncontrolled proliferation of B-lymphoid precursors arrested at a par-ticular stage of B-cell development. Precursor-B-ALL is classified mainly into pro-B-ALL, common-ALL and pre-B-ALL. The Common Acute Lymphoblastic Antigen CD10 is the marker for common-ALL. Objective: This study was aimed to examine the ...
متن کاملProptosis as Initial Presentation of Acute Lymphoblastic Leukemia in an Otherwise Healthy Child: A Case Report and Review of The Literature
Acute lymphocytic leukemia (ALL) is one of the frequent malignancies in pediatrics and involves bone marrow and extramedullary sites. Proptosis as extramedullary involvement of leukemia usually present in acute and chronic myeloid leukemia. It is extremely rare for ALL to present initially as proptosis. Here, a-21-month-old boy was presented with proptosis without any associated symptoms except ...
